Activation of decoders

I have for the past year been trying to have the problem with my account solved and am refunded an amount of R 5839.60 owned to me by MultyChoice. I own two decoders with smart cards, a HD-PVR (decoder serial number P400280179) and DSD 1131 decoder. (Decoder serial number U201931952). I should be paying for a Premium and Extra view service since February 2012 with insurance coverage on both decoders.
The problem is that Decoders have been added and deleted from my account with the result that I have been paying anything from R 727.22 to R 3248.98 per month.
I have contacted your Call Centre via phone (on various occasions at considerable expense) and visited your Randburg Office and have been promised the following without any success:
a) My account will be credited and the amount owning refunded - Has not happened.
b) Decoders are being loaded via internet by someone who has my id particulars. Have now been informed that an email request is received from a Yahoo email address to load the decoders. It is not my address.
c) I have been requested twice for a password to be supplied by me if I need any changes - did not work. I only found out that the password is not really needed
