Account debited without written consent

I currently pay via internet banking:

In February I was contacted by a lady from Multichoice informing me of the positives of going over to debit order, I confirmed with her that it might be a good idea and she must email me the forms which I'll look over and sign for them to debit my account.

On conversations with family and friends who have had issues with debit orders from Multichoice I decided to refrain from going the debit order route and contacted a moderator on the DSTV forum, whilst doing that I find out that MC have already debited my account and removed an amount of R652.72?

The moderator on dstv forum MC CCS KaraboG reply:

I can confirm that the debit order details were removed. I'm still busy with the investigation of why the debit order went through without you sending the completed forms back to us and I will feedback to you once I am complete.
Once again my apologies for the inconvenience caused.

I checked my account this morning and see NO amount of R652.72 has been credited or returned to my account?
