Incompetent Personnel at the call centre

I called to find out how i can terminate being a dependant and the lady told me i should send a letter and my ID copy then all will be taken care of. Now more than a month later i'm being told that she told me the wrong information and my mother needs to be the one to send a termination letter. I'm going to have to pay penalties now when joining the new scheme and who should be liable for that? Hire people who knows what to do and stop sucking people's blood for profit.
