Transplant done without a PMB Form

I am very unhappy with the way Discovery Health handled my transplant which took place in August 2012. According to Discovery Health they did not receive the PMB form that was needed for the transplant and as per my doctor that did the transplant the form was send to Discovery Health a couple of times. I am also unhappy because I still have Lancet accounts to pay which according to me Discovery Health had to pay 100% for the transplant. Discovery Health did receive motivational letters from my doctor but still I owe them money for the transplant. I want them to follow up and give me answers.
