Awaiting payment for claim since 2012

I have a patient who has had a stroke, and his condition was listed as PMB, which authorised 26 sessions. However, I am still awaiting payment for incomplete payment from 2012. He has since then been authorised for additional 26 sessions and I have incomplete payment from March 2013. After sending numerous emails quering this, as well as the patient 's son quering this, GEMS is requesting more and more documentation that claims have been sent. I have been transferred 4 times today on a call to query this and I had enough. These sessions have already been approved beforehand, how can they now pay short and also say the claims of 2012 are too far back.
