Where can one begin! Company send through a new application for med aid, attached ID Copy, Birth cert and payslip on 06.03.13. Got a reply form sizwe 11.06.13 (3 Months) later stating that we did not stipulate which option but the option was send on the same day requesting membership! On the 13 June 2013 our employee still did not receive any med cards to date, whereupon we emailed Sizwe again requesting the cards, they then send us a membership certificate and to no surprise the surname where spelled incorrect! Various emails were sent together with ID copy requesting for his surname to be corrected. Yesterday I phoned, faxed and email Sizwe again, our employees wife is in hospital and still no card nor amended membership certificate, we have been paying Sizwe since March 2013 for a SERVICE NOT RECEIVED!!
