I was hospitalized in 2012/06/20 and had Pre-authorization from my medical aid Polmed, Pre-authorization nr QH51046515 and the Dr did not claim at the right time frames and now is thinking I must foot the bill because of his mistake. My Polmed nr is 0118503.
