Fnb unauthorised debit orde

First direct issued a debit order against our Nedbank account with reference no. FNBRTCREV... We have not given consent for any such debit. The number that was given to me by First Rand to phone 087 736 3206 are not answered at all. How does something like this happen and where did they get hold of our banking details??
