Unauthorized debit orders / just a sales scheme

On the first of March 2013, there were four debit orders from my account.

I don't understand why this is as as I have only authorised 3 of those debit orders. (One of them in fine print)

The total sum of all these debit orders being R1698.50 which has by no means been communicated to me, with an extra R792 for joining a gym of which I have been a member for over 4 years.

I have an acceptance letter and policy that says that my premium is R914.

I have been led to believe by these and by your sales consultants over the phone that this premium included the savings contribution of R200.

Furthermore,

I was led by your sales consultants to believe that there would be a premium of R147 for Multiply in order to facilitate free gym membership and there would be no further fees. I was also told that I would be paid an amount of R430 a month if I were to gym more than 13 times a month and no mention of a debit of that amount.

I have subsequently also been forced by Virgin Active to pay a joining fee of R792 where I was told over email and phone that no joining fee would apply. I have also been debited an amount of R437.50 which I can only assume is for monthly gym fees.
