Terrible service from Old Mutual

I have been unemployed from August 2012 till March 2013. on 20 June 2013 my broker advised that the policy can be revived. So i completed the forms and mailed it 21 June 2013. Since then i have had no response. On 3 September i queried a premium that went off my acct and later found out that it was not from OM. after asking what happened to my revival, i received a response saying that they are waiting for the forms that i needed to complete which wasnt sent to me. I have been quering since. I have been paying for this policy from August 2000. Today i received a response that the add forms i needed to complete was sent to the broker and was never sent to me. To date, i have been asking and asking and no response. All this is cos i stated on the revival form that i had suffered from depression for a period after my day had committed Suicide. Either i need a new broker or move my policy to another insurance that will give me a better service.
