I cannot access my RETIREMENT MONEY!!!

I have a retirement annuity with Momentum that has expired in July 2013 already. On maturity, I gave the instruction to momentum to give me the maximum lump sum allowable in cash, and to transfer the remainder to my METROPOLITAN COMPULSORY LIFE ANNUITY. However, momentum informed me that they cannot proceed with the transfer until such time that METROPOLITAN has sent them an application for the transfer. I have gone to METROPOLITAN's offices in Rustenburg where I filled in forms with the consultant there who assured me that the application will be sent to Momentum. However, after a month elapsed, and with no solution, I started to call your (Metropolitan) call centre in Bellville countless times, but with each call being told that the application will be sent as they can see my forms that I have sent from my Rustenburg Branch. BUT, to date, I am still waiting! Momentum is waiting for the application from Metropolitan before they can give me my lump sum. PLEASE give this matter your utmost attention as I have really been patient with you people but clearly, that does NOT help! It is UNACCEPTABLE that at my age, I HAVE TO BEG FOR MY HARD EARNED MONEY!!! My patience has worn out
