Incorrect arrangement

On the 19/08/13 i called african bank to make an arrangement and i spoke to the lady called C Letsie and before i could tell her how much i wanted to be debited my call got cut off, two minutes later i get an sms that my account has been debited. i then called african bank again to make an arrangemnt and am told there is already one made for month end. how can she conclude an arrangement without my knowldge or agreement. i want dis girl to be dealt with and i want an apology from her.
