Promised a debit order will be paid, but returned

On the 19 June 2013 I has insuffient funds and my Hollard Policy was returned. On the 20 June 2013, I went to Johannesburg Branch and spoke to Tisetso and her Supervisor. I made an arrangement that I will adjust my account and they should pay my insurance policy. I explained to Tisetso for the importance that the debit or must be paid otherwise my policy will lapse. Tisetso and her Supervisor gave me assurance that the policy will be paid as long as i will ajust my account on the 20 June 2013 before 12pm. My account was adjusted around 11am. On the 21 June 2013, I got the confirmation that my HOLLARD INSURANCE WAS RETURNED. When I called Insurance, they confirmed that it was returned. I am dissapointed that how can Standard Bank do this me. I am a loyal customer, and had my account for years. I always provide for my debit orders hence my BIR score is high. I feel like closing this account and taking it to the bank where they will have customers interest at heart. NOW I AM WITHOUT INSURANCE!!!
