Telkom reason for my bad credit

Last year I opened a Telkom Simple II account, R499 pm. The first bills with pro-rata and it was payment in advance. Second bill was almost the same, and the accounts said that it is pro-rata. I explained that pro-rata can't be billed twice. Eventually I found out, that I was billed twice, the Simple Account (R499) and TI-AllAccess 10GB (R130). The called was also billed twice. I tried to fix this, but no luck as the buck was passed from person to person.
I eventually cancelled all accounts, and paid everything, even some reactivation to a normal line, which I also not requested. Final statement was mailed. stating no money owed. Months later, when I needed a normal line, I discovered that R158.15 was still outstanding on my account. I eventually got a credit and on Monday the Canal Walk branch confirmed my account is cleared.
This week when I wanted to apply for Vehicle Finance, I discovered that I have a default against my name, create by Telkom. I am unable to buy a car as a result of Telkom that billed me twice and dragged their feet on this. Telkom already owe more than R700 and now this.

Can anyone at Telkom please clear my name at the Credit Bureau.
