Contract Cell blacklisted without my knowledge

I bought a blackberry 9810 in February 2012 on contract from MTN in Queenstown. A week later i had battery problems, MTN in Century City then replaced my cellphone but in April 2013 i had encountered another problem. They once again replaced my phone which i used up until 2 weeks ago when i suddenly got a message saying "phone not allowed MM6 " after enquiring i was told that the phone had been blocked and blacklisted. How is this even possible if i got the replacement cell directly from MTN. Yet my debit order for the contract went through yesterday. I spoke to Pam (MTN Canal Walk) as well as Bianca, Makari, Busi and Luwene (Call centre MTN). I was requested by MTN to get an affidavit which i did and forwarded it to Pam who told me it would take 4 working days to unblock the phone to date the cell is still blocked and Pam still has not acknowledged or responded to any of my calls. THIS IS POOR SERVICE!! To my knowledge the only time a phone can be blocked is if it is or lost or requested by the owner for it to be blocked. I have NOT had my phone and did NOT request MTN to block my phone. IMMEDIATE assistance from a competent person would be much appreciated
