Requests that are not carried out and Bad service

Can some1 please tell me why my account is debited from wrong banking details. I have called in May and requested my details to be changed which I was told they have been done, and asked the lady I was speaking to if the debit order will go through on the new details, she said yes . come the 25th of May the debit order went through on the old bank details and it returned unpaid which I was charged R150.00 for the rd at the bank, I also went and made a cash payment. Again in June the same thing happened and I was charged another R150.00 at the bank for the debit order that returned. I want some1 to fix this mess cause I am not going to the bank and waste my time for the mistake done by incompetent people who do not know how to do their job. I want my account rectified asap, and I also want the bank charges credited to me as I have done what I?? m supposed to do as a responsible account holder, and I I receive and sms or my lines get suspended because the account is not paid which is not my fault..
