Unable to activate membership for over 2 months

In November 2013, I completed all the necessary forms at fourways planet fitness with Themba Mguni to join the gym. I had to wait until the end of November as my Discovery vitality was still linked to Virgin active. This was deactivated on the 30 November (as confirmed with discovery). According to my bank statement R 965 (3 December 2013) was debited from my account for joining Planet Fitness.

To date, every single time I have visited your gym I have had problems because according to your system my membership was not activated. I have spoken to Themba at every visit and I am finding to be highly incompetent as he has still not being able to tell me what the problem is.
