Unknown Erro

Went to virgin active to night to use the gym. Was stooped from entrance as they say that a debit order for 99.99 was not paid with an unknown error. I know there is more then sufficient in my account to cover payment so what does unknown error mean,
Plus as they checked it shows that they had not cancelled my sons membership even though he has been in twice to get it cancelled They tell me that we need to pay a cancellation fee.
Why was he not told this when he went in. Why does the receptionist expect to sort out the unknown error on their system. Spoke with the acting manager but no help there.
I've been using Virgin Active for three years why is the administration of the company so poor?
Really disappointed with the service.
