No statement - disconnected

On the 29th April, I paid the TOPTv rental for May, we called in and the gentleman on the phone that day was super, he reconnected us in a matter of minutes. 4 Days later, toptv discontinued our service, after calling in they said we had missed a payment, and they had run a statement that was sent to us (last statement was in December 2012) and therefore could not reconnect. I called back and asked them to explain it to me, with which i was told i am not the account holder so they cant talk to me, When my husband called they said we will need to pay the outstanding amount> that we are not even aware of!! An hour later some one had called him for us to cancel our subscription. I would really like to see the proof that the statement was sent as well as where "we owe the money"
