Debit orders not authorised

I have been getting debit orders going off my account and on the 19.06.2013 i decided to call all the suppliers to see what im getting debited for. I called the standard bank number as per the bank statement and they refered me to the 0861106143 number which then refered me to 2 different other numbers. I phoned the Milc they informed me that vodacom offered me a phone in 2009 & Im getting charged for services on downloads which is a benefit that came with the phone I never received or so they say it was declined. I called their call centre and the agent wasnt helpful as she told me that i was suppose to call them to cancel it. How is it that the cellphone offer was declined but they still managed to debit my account when i never received nor used any of their services. Im not happy and feel robbed, i want my money back.
Debit Order reference MILC EM-825129-3.
Amount debited Monthly is R19.00
I have requested the call centre agent to cancel this debit order and i want my money to be refunded.
