Cancellation of Vitality

I've had the Vitality Benefit for about 2 years now. Requested 2013 that my Vitality benefit should be cancelled via phone calls to there offices and it was said that it will be cancelled. Nothing happened after that phone calls and I sent emails to the info centre regarding this matter, but nothing has been cancelled and no effort is being done to communicate with me. Very unhappy about this and the fact that the debit is still going off on my salary where I asked that this should be cancelled about 3 months ago.
