Phyllis past and no death claim was issued

Mbasa Elford Phyllis Tyilana started working at the Courtyard Hotel in 2008 and he past on in 2010.There was supposed to be a death claim from that hotel. The family tried all directions to get Mbasa`s death claim. When we called several times in 2011, 2012 we would receive rudeness from an employer by the name of Herald. This Herald guy had come to the house to demand Mbasa`s death certificate which we didn't give to him please help us
