Incompetence, no reply to my uif application

I called quest on the 27-03-2013 requesting uif forms, I was then given an email address to supply my personal details. Which I did and I was told I would get a respons via email... Which I never received... I emailed time and again following every call I made to quest... Was promised response which I Never received to this day. This morning I called twice, spoke to Stacy who promised to call me back and I'm still waiting.

I have been calling almost everyday with empty promises.
I'm forfeting my uif funds next week. Can someone please help me ASAP so labour can process my application before I forfeit it... PLEASE
