I was employed by Quest during 2004/5/6 for FNB. On 03.01.2013 I wanted to submit my documents at our Labour Offices as I then started maternity leave, but was informed that Quest never submitted a UI-19 form in 2005 which they need before I can apply for UIF benefits. I phoned Quest HR/Payroll in JHB and my call was directed to a lady who could hardly communicate. I asked her if she could please send me a UI-19 form and her response was: "They will send it to you when it's ready". So I asked her since she's not asking for my details when will they then send it as they never sent it from 2005 and it's 2013 already, how long will I still have to wait? So she gave me an e-mail address to send my details to: [URL Removed] and I told her that that was not a valid e-mail address so eventually got the address [Email Removed] which I then used to send my details to. After a week I received no response and called again, and again she said they will send it when it's ready. Well, I even spoke to Lisa at Cape Town and still nothing! It's more than 2 months now and no one has even bothered to get back to me and I just have to have no income. (Supplier name corrected by Hellopeter.com)
