Transaction on my cheque

When ever I purchase online using my cheque card I always get a OTP confirmation through my cellphone and the purchase only gets activated after I have confirmed it through the OTP Number. But yesterday [17/04/2013] I got an sms notification of a transaction going through which I never actioned and what even irritates me the most, is how was that transaction authorized without my approval through the OTP. Firstly its a transaction on my card and secondly FNB allowed to go through without an OTP confirmation from me.
