No Beneficiaries!!! I die, my wife starves

I asked for my beneficiary to be added on the 10th of October 2013 and there was no luck. On 01/11/2013 I tried sending it again and to my surprise - no response to date... I am sending emails to the speedy email address that always handles my queries all the time. I don't know what to do anymore. I was told I cant add beneficiary without that form I filled and sent to Customer Care but no luck!!! Please advise what is going on???
