Payment break that I didn't authorise

A while back I complained about debit order that was not activated by african bank.then my account went into areas but wen I contacted african bank I was told that I applied for payment break.the guy who was helping me told me that he ws still tryn to find the person who authorised it. I had to make arrangement of payment n I'm paying R200 extra in my account for that. I still wanna know what happened n Y I have to pay for africans bank fault.plz contact me @ work if I don't ansa my cell.011 7415318/13
