Cheque Account Opened without my Consent

Today I received a cheque account statement? For what? My banking charges are through the roof as well. I need this cheque account removed. I did not sign for anything and I did not request a cheque account. I only have a Smart Savings account and a Smart Savings Debit Card. That is it. Point. Finished. Done. Reimburse my banking charges which you without my consent and remove the cheque account from my name, or I will transfer my money out of this account and go to another bank. That's it.
