Unnecessary billing

I settled my loan on 30.07.13. Phoned to confirm this on 01.08.13, spoke to Angelo. I also made sure to ask whether the 2 x R99.99 insurance premiums against the loan would be stopped. Angelo confirmed that no debit orders would go through for these again. However, despite me following up on this, on 30.08.13 the two debit orders were again deducted from my account.
I demand a) for the debit orders to be immediatley cancelled and b) to be reimbursed for the deducted amount of R199.98. Please advise URGENTLY.
