Duplication on UIF Declaration

I have been informed by the Labour Department that OMF, is sending duplication UIF declaration and this is making difficult for the department to process and finalise my claim. Someone very incompetent is sending this declaration many times and the system is failing to have an accurate calculations.
I need these to be sorted as soon as possible and can OMF or whoever is responsible in submitting UIF declarations, stop duplicating the submission. i also want the updated U1 .19 form, to be send to today by email to the following address [email protected], who is awaiting this documents.
