Someone is lying. Mutual & Federal or Melomed

My father had a heart attack and died 19 August 2012. I called swiftcare (free service part of a M&F insuarance policy) for assistance and they send a melomed ambulance. I then get a bill for this ambulance, which was supposed to be paid for by M&F/swiftcare. After a month of back and fourth!! Someone from M&F comes back to me to tell me that there was a mistake and i definately should not have gotten a bill from Melomed. Now more than a year later i get a registered letter from Melomed saying their next steps will be to take my dead father to court. Great timing considering its a week after the anniversary of his death. So my question is this. Who is LYING here? Melomed who is pretty convinced they have not gotten paid or M&F who assured me that the bill was taken care of. What makes it worst is the people in the accounts department at Melomed has bad attitudes. They think they can talk to you in any tone they choose and when you react they want to give you a lecture on how they should be addressed with respect. Yet they have no respect for anyone. After a year of harassment and then disrespect from staff and they expect people to be nice to them?!?! Really!?!?
