Double billing BIS - worse after visiting branch

I send faxes, I visited both Lephalale and Bela-Bela branches - every time I try to sort out the mess on the account it gets even worse.

This is a copy of the last fax, sent on the 23rd of January:
This is absurd! I am trying for 4 months now to sort out this account - get your house in order! I WILL NOT pay any reconnection fees - you must fix your statements, I can not be held liable if you do not fix the statements & debit order amount in time!
I signed up for 3 identical contracts in December 2010. Refer to the contracts to fix your errors please!!!
I pay the following on each contract:
1. SIM Insurance R3.51
2. Clip R7.45
3. Telephony R87.72
4. Monthly Device R25.44
5. NEW: Insurance 3000 R55
TOTAL: R179.12

MONTHLY TOTAL ON MY ACCOUNT: 179.12 x 3 = R537.36

I did not even get an acknowledgement from the accounts department, my monthly bill for these top-up packages now is R1311.
