2014 PMB Claim requiring LOM

I have a PMB authorisation for Lancet which is renewed annually, I've in December 2013 had my bloods done which was my last 4th service for the year, on 16 January 2014 I had my routine bloods done for a script renewal, now I received a processed statement that confirms non payment as a LOM is required, why is Medihelp now running the 90 day period which applied in 2013 to the new year, because the benefits for 2014 starts anew with only 2 routine bloods annually for codes 3816/4429. I'm not going to request my Dr to complete and LOM, I've a 2014 Basket Care which in it doesn't mention anything about 90 days period runs from the previous year, why then do I have new benefits if they run concurrently, the CMS, Consumer Protection Act and the Investigative Media will agree with me on this one. Lastly I don't even receive statements of paid claim, only processed claims, Lastly MH should not attempt paying this claim from my available savings.
