FNB used 3 different made up names for my accounts

I have been an FNB client for a few months. When my credit card was delivered today I realised after a while that it had the wrong 1st name printed on it (not my name, but my surname was correct). I then discovered that the initials on my cheque card, which I've had for many months but I have never looked at closely, was also wrong, but different again from the credit card name! That means that FNB somehow has 3 different names connected with my accounts (my real name, and 2 different made up names which are not connected with me in any way). They have my ID number, a copy of my ID book, my proof of address, all with my real name on it and they ask for my name every time I make a call... So how can such a 'mistake' occur? I would be very interested to know if this happens frequently, it seems an incredibly ripe opportunity for behaviour, and certainly has made me loose all trust in FNB's safety... scary and more than a bit unprofessional.
