LEVEL LIFECHL deducting R28.50 for NO REASON

I received a DO from my bank Standard bank for LEVEL LIFECHL of R28.50 i have No Insurance with ANY company. YET standard bank allowed this DO. i have no given any company permission to Debit my account.
i a highly as Standard bank cannot help me as i need to pay them to give me the details of this DO as well as pay them to do a Stop Order... I want money refunded and this DO's to STOP ASAP!
