Why are Moto Health not paying

I have just completed a second batch of dental treatments, the first batch was pretty much all paid by moto health but this treatment has mostly been refused payment. As the first treatment was very similiar to the second, only top teeth instead of bottom. After contacting them I was informed that no pre-authorisation was obtained. As I have a letter from Dayalan Govender (ref 164933302) confirming acknowledgement of my request for authorisation this is funny that they have no reference to this. I also have e-mails with reference numbers from them also on record of this request. What is even stranger is that I was told that pre-authorisation is required if the work is carried out in Hospital not a walk in dentist that is covered under "day to day' and is limited by an annual limit.
Can someone who knows what this policy covers please pay this second claim.!!!
