Debit reversed when funds available, unwanted charg

Hi, I've deposited r180 on the 4th of september for my budget insurance. On my bank statemant, it showed my deposit and then my budget insurance debit gone through. Now I've received a statement from budget saying my debit did not go through and my policy has lapsed. I've got charged 57 rand for a incidental fee when I was never in arrears. Now my account is on a negative 287 rand and my policy has lapsed, when on the 4th of september I had deposited r180 for my debit to go out and my account to be in order. My bank statement shows my deposit goone through and my budget debit gone through. How can everything get so messed up
