Asked for insurance

My husband bought a Samsung Galaxy S3 on the 03rd of April 2013 cash @ MTN Upington. He told Tracy-Lee that he want to take out insurance on his phone and she must please give him the documentation to fill in. She gave him papers to sign and he got his phone. End of July he went back to MTN Upington to put in a claim. They told him that he didn't take out any insurance on his phone and cannot claim for any damages to the phone. He told them that he did take out insurance @ Tracy-Lee. She said that she forgot to give him the insurance application to fill out and therefor he don't have any insurance on the phone. He told her that he DID sign papers (what we thought was for the insurance). She just said "sorry, that wasn't the insurance papers you've signed, so you don't have insurance and can't claim".

This is REALLY not my problem if they can't do their job properly. We want the phone fixed, that's why we wanted insurance for accidents like this!!!
