Debth on Disconnected Decode

I have a problem which was sent to me that i'm owing the decoder that i requested to be disconnected on the 09 June 2013. After i pay R540.00 on the 8th June 2013. On the 09 June 2013 (ref# 133610323) i called to reconnect the DSTV and ask the smart card number 4119324769 to be disconnected. the consultant i spoke to said for July i will only pay the difference since the smart card number was disconnected.

On the 28 June 2013 i called requesting the balance that i should pay (Ref#135570693), and the lady found out the smart card number which i requested to be disconnect has been connected without my request on the 10th June 2013. And she said she will resend the e-mail to finance to reverse the amount for the disconnected decoder which is 4119324769. And i paid the difference on R198.00 which she said i must pay. And today i get the statement that say i have to pay R104.70 on the smart card i requested to be disconnected on the 9th June 2013, which on your record shows that someone connected it on the 10 June 2013 with the Username 03/SA/BTS. I will like this to be investigated thoroughly as i haven't ask for the reconnection on the smart card number 4119324769.
