Don't receive invoices

MTN has locked my modem (I have a data contract with them) because I haven't paid. However, I haven't received an invoice. I have phoned them about 5 times requesting that they send me an invoice but nothing happens. I will pay immediately if I know what amount to pay and into which account! Everytime I phone the call centre the assistant promises they will "send the request through to their supervisor" and then nothing happens. When I phone back and ask to speak to the previous person who assisted me, they say that is not how they operate, a new person will assist me right now, or the request will be noted and forwarded to the supervisor. On Thursday I requested to speak to the supervisor, and then I was cut off (I'm sure it was on purpose). I actually WANT to pay them but they don't assist me to so. Please help. I would actually prefer to terminate the contract if possible. If not, I will honour the agreement and pay my accounts - but I cannot do so if I don't receive an invoice. I do not even know my account number since I've never received any invoice or other paperwork from them.
