Contract not cancelled as requested NUMEROUS times

My contract for number 074 156 5503 was supposed to expire on 30 Nov. Now I have been credited again for the month of Dec, i.e the contract has not been cancelled as per my NUMEROUS requests.
I requested a cancellation THREE times. Once in mid October, again at the end of Oct and again mid Nov. From mid Oct I was told that the last payment would be collected at the end of Nov and that the contract would expire (i.e be inactive) from 1 Dec. This is now not the case. I refuse to pay for another month of a contract THAT I CANCELLED THREE TIMES.
