Charged 3 Months fee's and still no refund

I added my wife as beneficiary on 20 June 2013 on my Bonitas BonCap option. The charge per month, for her, should be R 1, 261. In July my account was debited R 3, 783. The call centre agents (3 calls made) said it was a late starter penalty fee (They start cover from 1 May 2013).
At no point did Bonitas mention that I would be charged this fee. While the call centre agents are helpful NONE have been able to tell me when I will get my refund. They very quickly removed my wife as a beneficiary but have given me no feedback about my refund. The agents that were meant to call me back, have not.
