Failure to provide proper service

I applied for membership and was placed on a three month waiting period, when queried the response took two weeks to come through stating they needed my certificate from a previous medical to be on cover immediately, after sending the certificate through dispite numerous calls to the call centre and i received a response from them stating the waiting period would still apply. i have terminated my membership and requested a refund as i was a non-starter. they said it would be done in three days of termination. they send me a sms saying i need to fax my bank statement and i.d to them, when i attempt to do so their fax line doesnt work. i have spoken to over 10 consultants regarding the fax and refund since the 09/04/2013 and all they say is we will escalate to finance and respond but i have had no resolution till now. they refuse to assist in processing the refund. when i filled out the application i selected the option for the refund and debit to go off the same bank account so i dont understand why they say they need a statement to be sent to them when they debited the very same account without needing the bank statement. i just want a refund and never to deal with Bonitas again!
