Charged for unrequested services and slow response

My February invoice included a charge for 3 GB data bundle that was never requested nor used.
1 GB data bundle was purchased and paid for in January 2013.
Phoned call center on 4 Feb to follow up - call got passed on to credit department with promise that it would be resolved/responded to within 24 business hours. No feedback received - on calling credit department on 8 Feb, was informed it would take another 7 days for credit department to send feedback. No feedback received. Phoned again on 14 Feb and told issue was still "pending" with no progress.
Service has been appalling and billing charges amount to almost R500 extra for something not requested.
