Change of ownership not completed and no feedback

I changed my contract from my mother's name to mine a month ago along with the upgrade the contract i was using was due for.
The upgrade was done fine.
Then my mother received a bill in her name in her inbox, so not changed to my information after the whole saga of collecting salary slips, bank statements and proof of residence.
On top of that, the manager, who was well aware that i had changed from a Blackberry phone to a Samsung, sneakily "omitted" to advise me that i needed to request a cancellation of my BES service as i OBVIOUSLY would not need this if i no longer has a Blackberry handset. So i was charged for that in the latest bill (sent to my mother)
I went into the store last week to get this corrected, the manager saw me and avoided me after hearing my complaint and i still have not had a response from him. The person who attended to me was a temp employee in the store and the manager had helped me the last time so i needed him to fix his mess.
