Contract Expired - Still Paying!

My contract was meant to have expired end of November 2012. In November, I sent through a cancellation notice to make sure that my contract must not continue. Result I was billed in November.
In December, 8ta says they have no record of a cancellation notice, I send through another cancellation note. Result, I get phoned saying that my contract has been migrated to pre-paid and I will definetely not be charged again. Actual Result: I still pay for December.
In January, I phone numerous times to ensure that I am on pre-paid now. I get told numerous times I am not prepaid and I will not be charged again. However, I am billed again for January.
What must I do to cancel my contract and what must I do in order to get my money back for December and January? Not withstanding the endless hours I have spent on 8ta support lines, and people promising and apologising that this will be sorted out, but NEVER is.

New contract customers beware, it seems you can never get out of the 8ta contract hold on you!
