Branch Lost my request to cancel contract

I went a month in advance to the branch to cancel my Data contract. Just to receive another bill at the end of January for a contract that should have ended 04.01.14. When I phoned the call centre they said there was no record of cancellation on the system. I then went back to the branch and found out they lost my documents and no one knew where they were. I was assured they would sort everything out and credit the invoice. I have still received no correspondence and have now been debited of my account again. The call centre cannot help me and none of the branch co tact numbers are answered. So I will have to go back to the branch again. And above that I have been struggling for more than a month to get a credit for almost 10 months of BIS I was charged for while I have a samsung phone.
