No cancellation after 30 days notice given

I have had no ADSL service since november 2013 (ref#603CWK061113) I requested a cancellation on the 17th November and was assured it would be cancelled by December 17th 2013. The line has still not been cancelled nor has the fault been fixed. After about 3 different requests to log a billing dispute as it is unfair to be billed for a service which is not working I have had zero feedback from Telkom. I am fed up and just want all my services cancelled so I can pay the final invoice and be done with them. I am fed up with the disgusting service I have received. Please can some one help me terminate everything and be done with this terrible company?
