Unauthorized New Contracts

In June this year Vodacom phone to say that am I aware that two new contracts were opened in my name, which is 0716836170 and 0716836185, I was told to send an affidavit to say that I did not open those lines which I did, but still I get calls and my other lines were blocked due to the fact off the lines that was opened on my name. They say Cell Direct opened those lines on my name. I asked the bank to reverse the amount of R660, as I cannot pay foir something that I did not apply for, I need the charges off the debit order reversal to be refunded to me as well as the account to be closed and cancelled. The person responsible must be taken to task as I did not apply for the contracts. My credit report will be affected negatively, I need written confirmation that the account was opened and is now closed.
