Delayed Feedback for a claim

I submitted a claim for an immunisation in August, only to be told that I need to submit proof of payment.
They had an invoice with a big PAID stamp on it, but I was told that this is not good enough. I therefore submitted my bank statement with the payment showing clearly.
I received no further feedback nor payment. Eventually I followed up with my broker who then followed up on my behalf. Now I received feedback today that it has been rejected because I must submit an amended claim with a null balance on it (I do not even understand what that means) or a receipt. I paid with my cheque card and no longer have the slip. I do not see why it should be rejected on these grounds. Firstly I was not even told it would not be paid, I just received a statement with a reason code on it that said proof of payment required, then after following up I was only asked for proof of payment. Why does a bank statement not qualify as proof of payment? I joined Liberty after being frustrated with my existing medical aid supplier but am regretting the decision.
